If you need enrollment verification for CHAMPVA insurance please submit the CHAMPVA form along with your enrollment certification form to our office.

Please review the CHAMPVA School Enrollment Certification Letter here.

You are responsible for making this request in advance when you submit your Enrollment Certification Request form to the Veterans Success Center every semester.

Please allow at least 2 weeks for processing of the verification.
