Group Photo Booth Request Form

This form should be submitted if you are looking to schedule a time for your group, office, or organization to use the Photo Booth. If you are not looking to schedule a time for a group to utilize the Photo Booth, please attend the Peer Career Advisor Drop-In Hours. Requests must be submitted at least two weeks prior to the preferred date.  A staff member will follow up with you to confirm your scheduled time and provide further logistical information.
