Group Photo Booth Request Form

This form should be submitted if you are looking to schedule a time for your group, office, or organization to use the Photo Booth. You do not need to fill out this form if you are planning to attend alone, just stop by our office anytime during business hours (Monday-Friday 8:00am-5:00pm) and a staff member will assist you.ย 

Requests must be submitted at least two weeks prior to the preferred date. ย A staff member will follow up with you to confirm your scheduled time and provide further logistical information.
